Welcome to the Health Office at Saint Denis Saint Columba School!
Please read the following information regarding important policies and procedures set forth by NYS. If you have any questions, feel free to call.
Physical Exams - Grades K, 1, 3, 5, 7 - A valid Health Exam dated 9/5/24 or newer needs to be on file to fulfill the NYS requirement. Physical exam forms are due by October 1st.
Screenings (Hearing, Vision - near/far/color, Scoliosis - 5th, 7th Grade girls) not completed by your physician will be scheduled and completed by the school nurse at a later date. Any failing/concerning results will be sent home with a phone call to parents prior.
Vaccinations
* Kindergarten - Vaccination records in compliance with NYS regulations need to be submitted prior to the first day of school. Students attending SDSC are required to receive the same NYS required immunizations as students attending public schools.
* 6th Grade - TDap Vaccine is required for all students age 11 and entering 6th grade.
* 7th Grade - Meningococcal Vaccine is required for all students entering 7th grade
* Students with missing vaccinations will have a 14 day period to show proof of vaccination or provide a valid medical exemption from the vaccination.
In School Medications - (This includes ALL medications - including OTC medication, cough drops, Lactaid, eye drops (artificial tears/antibiotics), any lotions/creams) - All medications to be given in school require an order from your physician and a completed parent permission form. New orders need to be submitted at the beginning of every school year. Students that may be able to self-carry their medication will need to have an attestation form completed by your physician and signed by a parent along with the previously stated forms. All medication should be brought into school by a parent or a trusted adult. No medication should be traveling with your child on the school bus without a self carry permission form.
